- Provide direct patient care to residents in a skilled nursing / long-term care facility.
- Administer medications and treatments as prescribed by physicians.
- Monitor and record vital signs, symptoms, and patient conditions.
- Assist with care plans and report changes in residents’ health to the RN/physician.
- Perform wound care, injections, and basic nursing procedures.
- Assist residents with daily living activities (ADLs) such as hygiene, mobility, and feeding.
- Provide care for elderly and rehabilitation patients.
- Maintain accurate medical records and charting.
- Follow facility policies, infection control, and safety protocols.
- Graduate of an accredited LPN program.
- License: Active LPN license in the state of practice (compact/multi‑state requirements should be verified).
- Experience: Minimum 6 months LPN clinical experience.
- Certifications: BLS Card, Covid Card; negative TB test.
